Step 1:   Download the required document

Step 2:   Complete the form (either electronically or manually)

Step 3:   Sign the form, either electronically or manually. The form must be signed. Please do not type your name.

Step 4:   Submit your completed form to our RTO Admin team via: [email protected] with the subject line <Form Name - Full Name> for processing.
